1. Company Information

This website and all associated reports and data products ("Products") are published and sold by Transparency Project LLC ("Company," "we," "us," or "our"). By purchasing or using any of our Products, you ("Buyer," "you," or "your") agree to be bound by these terms.

2. Terms of Use

By accessing this website or purchasing any Product, you acknowledge that you have read, understood, and agree to be bound by these Legal Disclosures and Terms of Use. If you do not agree to these terms, do not use this website or purchase any Product.

These terms constitute a legally binding agreement between you and Transparency Project LLC. Your purchase and/or continued use of our Products constitutes acceptance of these terms in their entirety.

3. Data Source & Federal Authority

All reimbursement rate data contained in our Products is extracted from Machine-Readable Files (MRFs) that health insurance issuers are required to publish under the federal Transparency in Coverage Final Rule (85 FR 72158), codified at 45 CFR 147.211.

This rule, effective July 1, 2022, requires group health plans and health insurance issuers to publicly disclose negotiated rates for covered items and services through machine-readable files updated monthly.

Our data was accessed from publicly available MRFs in January 2026. We do not obtain data through any non-public, proprietary, confidential, or unauthorized means. All source data is publicly available on the open internet as mandated by federal law.

4. Data Methodology & Limitations

4.1 Extraction Process

Rate data is programmatically extracted from insurer-published JSON and GZIP files. These files are often hundreds of gigabytes in size and contain millions of records. Our extraction pipeline filters for specific CPT codes, geographic regions, and provider types relevant to each report.

4.2 Deduplication

Where multiple rate entries exist for the same provider and billing code, we retain the highest observed negotiated rate for that provider. This approach was chosen to represent the most favorable rate available to each provider within that payer's network.

4.3 Outlier Handling

Statistical outliers (rates that fall outside reasonable bounds for the given CPT code) are identified and excluded. The specific methodology and thresholds used for outlier filtering are disclosed in each report.

Transparency Project LLC is not affiliated with, endorsed by, sponsored by, or in any way connected to any insurance company, health plan, or healthcare organization whose data appears in our reports. This includes but is not limited to: Aetna, Anthem Blue Cross Blue Shield, Cigna, UnitedHealthcare, Florida Blue, Harvard Pilgrim Health Care, and Tufts Health Plan.

Use of these company names is solely for the purpose of identifying the source of publicly mandated data and does not imply any relationship, endorsement, or sponsorship.

7. Provider Data & Public Records

Our reports contain the names, credentials, cities, and National Provider Identifiers (NPIs) of individual healthcare providers. This information is derived from two public sources:

  1. Insurer Machine-Readable Files — Published by insurers under federal mandate (45 CFR 147.211). These files include provider identification information linked to negotiated rates.
  2. CMS NPPES NPI Registry — The National Plan and Provider Enumeration System, maintained by the Centers for Medicare & Medicaid Services, is a publicly searchable database of all healthcare providers with an NPI. This data is explicitly designated as public information by CMS.

We do not publish any protected health information (PHI), patient data, or information subject to HIPAA privacy protections. Negotiated rates between providers and insurance companies are not PHI.

Provider opt-out: If you are a healthcare provider and wish to request removal of your information from future editions of our reports, contact us at the email address listed below. Note that we cannot remove data from reports already sold, and your information remains publicly available in the federal data sources listed above regardless of any action we take.

While the underlying rate data originates from publicly mandated filings, our selection, arrangement, analysis, presentation, and commentary constitute protectable intellectual property.

We reserve the right to modify these terms at any time. Changes will be posted on this page with an updated "Last updated" date. Your continued use of our website or Products after changes are posted constitutes acceptance of the revised terms.

These terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions.

Any dispute arising out of or relating to these terms or your use of our Products shall first be attempted to be resolved through good-faith negotiation.
